ALL LEVELS SUPPORT COORDINATION AND CASE MANAGEMENT
At SN Connected Services, we provide support coordination for all levels of care. Whether you’re looking to focus on one area of support by connecting with specific services, or you require more holistic, specialised support, we can coordinate the services you need. The three levels of support coordination that we can include in your support plan are as follows.
Level 1: Support Connection
This level of support allows you to connect with informal, community, and funded supports. It is designed to help you get the most out of your care plan so you can maintain as much independence as possible.
Level 2: Support Coordination
This level of support will give you the education and skills you need to better understand your plan and how to use it effectively. We will collaborate with you to make sure a variety of different supports are included in your care. In turn, this can help you to better maintain relationships with those around you, manage certain tasks, and live more independently at home and in your community.
Level 3: Specialist Support Coordination and Case Management
As the highest level of support, this service is designed to help people who have complex support requirements and are in need of specialist support which includes crisis resolution, assistance to overcome barriers to service delivery, coordinate and collaborate with multiple services and stakeholders.
