How It All Began

SN Connected Services was founded by Registered Nurses Caroline and Kim, who have been colleagues and friends for nearly two decades. They started the business to help more people access person-centred, tailored, comprehensive support, taking a more holistic approach to their care to ensure they access and navigate the services they need. SN Connected Services has a team of exceptionally trained, highly supported staff who are deeply committed to the concept of choice and control.

Specialist Nursing Connected Services (SN Connected ) is a dedicated specialist support coordination and case management company helping people across Western Australia to access more person-centred planning and support.
Our specialist support coordination and case management services help clients overcome barriers and complexities in service delivery by using a holistic approach. We collaborate with clients, families, carers, providers and a range of stakeholders to foster meaningful and exceptional support.

  • About Kim

    Kim is a RN qualified in both adult and paediatrics nursing.


    Kim has extensive experience in paediatric intensive care and has held many positions in staff development/education.


    Kim keeps busy with her 4 adult children, she enjoys competitive swimming and the odd fun run.

CAROLINE

  • About Caroline

    Caroline is an RN qualified in both adult and children’s nursing with extensive experience in critical and complex care.


    Caroline has held numerous clinical, teaching and senior management roles in both Australia and the UK.


    Caroline is mum to a grownup son, she is very active, outgoing and energetic. She loves coastal walks, the beach and travelling.

  • About Lisa

    Prior to emigrating to Australia in 2010, from the UK, Lisa spent the majority of her career in hotel management and then ran her own business for the last 10 years.


    Since arriving in Australia in 2010, Lisa has held various roles starting as a support worker, gradually moving into management of rostering and recruitment. She still regularly cares for a young lady with Cerebral palsy who has been a bright light in her life for the past 5 years.


    “I completed a Diploma in Community Services last year and decided I would look for a more challenging role that fully utilised my experience and was more “client” focused. I joined SN Connected in July 2022 and, whilst certainly challenging,  I’m looking forward to the journey ahead, seeing this amazing organisation flourish and grow and continue to make such a huge difference to their clients' lives.”


    Outside of work, Lisa is married with twin boys aged 21. She loves to spend her downtime tending to her garden, considers herself a “crazy dog lady” and loves to paint!

  • About Sue

    I have had diverse career from working for a Cross channel ferry company to a Registered Training Company teaching Cert II and IV in Fitness.


    I have a background in Business Management and Development and have developed and operated two of my own businesses.


    I have a complimentary range of skills including administration, business management and training across private and commercial enterprises.


    I moved to Australia from the UK in 2006 and I worked within the mining industry in Perth as a manager for 4 years, before opening the Registered Training Company in 2009.


    Outside of work I go to the gym, kayak, spend time with friends socialising and with my beautiful family and two fur babies.

Zuzana Racekova

Specialist Support Coordinator/Case Manager

  • About Zuzana

    As a Registered Nurse specialising in case management, support coordination, training, and education, Zuzana has experience across an extensive array of nursing specialties, including disability and rehabilitation. Zuzana supports clients and their families of all age groups with complex care needs, clients with spinal cord injuries, traumatic brain injuries, and mental health needs. She is passionate about improving people's quality of life by supporting them on their life journey, applying a holistic approach, building trusting relationships, and collaborating with a vast range of stakeholders.
